Important for league to have your cell- so we can notify you of last minute game changes (our last minute notices are done by text only)
  1. As an athlete in such sports, I am familiar with methods of conducting volleyball and the inherent risk of physical injury, and I hereby waive, relinquish, and release any an all right to claim to damages which may occur to myself; I acknowledge that the league will not supply medical insurance (health insurance) and accident insurance and so if I am concerned about injuries, I understand that I will carry my own health insurance. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death. The risk of serious injury is substantial and people do get injured on a regular basis- this risk is real and does exist; I have read this release of liability and assumption of risk agreement, I fully understand its terms and I understand that I have given up substantial rights by signing it, and I sign this freely and voluntarily without any inducement. I choose to play and in fact, I would be playing elsewhere in open gyms and other recreational games if I were not playing in this recreational/amateur situation right now, so any injuries that occur in this program would have just as easily occurred in any other game that I would have been playing. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest supervisor immediately. I- for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Peaceful & Recreational Athletics, Inc. doing Volleyball as the Meetup group Rose City Adult Volleyball- their officers, supervisors, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners of premises used to conduct the event. Injuries- even serious injuries occur in adult recreational sports and I accept that risk (signature) .
  2. I realize that Peaceful & Recreational Athletics, Inc. is running these volleyball events, NOT the gym or school being used. Restated: the gym or school has nothing to do with these open gyms and games. Restated as an example: Columbia Christian High School, Beaverton Hoop, nor any gymnasium - is NOT running these volleyball events; rather those are just gyms being rented by Peaceful & Rereational Athletics Inc. and have absolutely nothing to do with this league; in no way can a venue be held liable for my injuries (signature): .
  3. I realize that confrontations, yelling, and any sort of anger are strictly prohibited. If any sort of conflict arises I will not confront the situation, but rather, I will remove myself from the situation immediately. If the problem is bad enough, I will call the league at 5033804539 - but I will not take matters into my own hands- (signature) .
  4. I realize that when I 'hold' a spot, there are no refunds or credits for cancelling. This means 1 or 2x per year you might pay $$ and not play. (signature) .
  5. I understand that the LISTINGS for volleyball are on the Meetup page. You will check THAT page for days, times and costs. Please do NOT ask Mikal directly (as Mikal often is too busy to look it up for you).   .
